Parish RV Repair Customer Waiver
o I hereby authorize Parish Rv Repair LLC to make the specified repairs/service quoted and discussed. I
understand that payment in full will be due upon completion, including additional supplemental damage
and/or service for repair charges.
o I hereby grant Parish Rv Repair LLC permission to operate/tow vehicle for the sole purpose of testing
and/or inspection. An express mechanics lien is hereby acknowledged on the vehicle to secure the amount
of payment for repairs and/or service thereto.
o Parish Rv Repair LLC will not be held responsible for loss or damage to vehicle/camper or articles left in
case of fire, theft, accident, or any other cause beyond your control.
o I hereby express my permission for the Parish RV Repair to use my premises to park their vehicle and heavy
equipment and assume the potential risks and liabilities associated with such activity on my premises,
including any damage to the property whether real or personal.
o There is a risk of spill of oil, grease, and/or other chemicals that may spill to my driveway, lawn, garage
and/or any other personal property.
o I acknowledge the risk of any possible spill of oils, chemicals, and contaminants that may be incurred
during the repair of my vehicle/camper and by which I assume the liability and responsibility of such
damage including any property within the premises. It is my responsibility for keeping away children, pets,
relatives, and guests during the repair/service to avoid or mitigate the risk of personal injury.
o Time spent in repairs through this method may take longer than usual. I accept and acknowledge any
unforeseen delays or expenses which may accrue for not having the service/repair completed on time.
o During repair, there may be unforeseen damaged parts that the mechanic might not see and might get
damaged. The mechanic shall inform me of these damaged parts and further assessment shall be made. This
shall incur additional costs for repair on my part should I ask for it to be repaired as well.
o If services cannot be completed within the specified time, Parish Rv Repair LLC shall contact the customer
concerning the delay and in his/her discretion, elect either to have services continue (understanding the
delay) or request that his/her property be promptly returned upon the request. Services/repairs upon that
time shall be paid for that period.
o I understand Parish Rv Repair LLC will order parts and start service/repairs as soon as possible granted any
unforeseen issues within shipping and/or materials that may occur.
o I understand there may be charges, including but not limited to the following: Towing, Storage,
Administrative fees, Appraisal fees, Parts restocking fees, Labor charges, legal and Recovery. A 4%
surcharge will be added to all credit/debit card transactions and a convenience fee can be added per.
o I also Understand that any charges not paid by the insurer are my responsibility, including but not limited to
the following: Towing, Deductible, Betterments or for additional work requested. I understand those
charges are due upon pickup/completion of work and service performed. A 10% fee will be applied on
invoices that are 30 days past due. For your convenience we accept all major credit cards and e-check payments.
o Unless prior arrangements have been made, I agree to pick up or have my vehicle delivered to me within
(2) business days upon completion, otherwise a ($55.00) per day storage charge will be added to the total
cost of repair/service of vehicle/camper. In the event I do not pay the charges owed necessary to remove
my vehicle/camper from Parish RV Repair LLC, I understand that I will be held responsible for all legal
fees incurred.
o I hereby waive my right to sue the Company for any and all damages, injuries, or death that I or my family
might incur during and in connection with the repair of my vehicle.
o I hereby declare that I am at least Eighteen (18) years of age and the legal and registered owner of the
Premises or have the full authority to sign this agreement.

Additional Terms and Conditions
Appointments subject to change, based on availability, location and logistics at the discretion/business needs of
Parish RV Repair. Cancelation of the appointment by the customer must be at least 24 hours prior to your
appointment to avoid a service call charge. Service Calls charge(s) are due when the technician(s) arrives at the unit
with or without completed work or diagnosis. If a customer cancels an appointment prior to 24 hour notice, a service
call fee is due immediately upon cancelation. If a customer refuses services or cancels service or avoids service less
than 24 hours notice, a service call fee is immediately due. If parts are required, these charges will be assessed at
the time of service. All part(s) that need to be ordered, will require a deposit. If your service requires the arrival of a
new part, a new schedule will need to be arranged for this part in which time and labor will be assessed for the part
installed. If other services or diagnosis are requested beyond the original service call, a new service call charge and
time and labor will be assessed.
Parts and Labor Warranty Disclaimer:
We Parish RV Repair do guarantee our work on a limited basis for time and labor. Time and Labor warranty will be
addressed on a case by case basis and discussed with the customer when issues or questions arise. If our service is
needed concerning a completed service repair we will address said repair and if further time and labor is necessary to
complete repairs over and above repairs already made it will be the customers responsibility to compensate for that
time and labor. If repairs are requested when we are performing repairs utilizing our guarantee of complete and
correct repairs the customer will be responsible for a service call fee and/or will need to coordinate a future
appointment to address additional service work. Parish RV Repair retains sole discretion of any Parts & Labor
Warranty Disclaimers, products and services in perpetuity.
Products and parts have a separate warranty through the manufacturer.
It is the customers responsibility to register products as instructed with the warranty information provided with parts.
We do not guarantee the warranty of manufactured new or used parts. If a product or part is defective or has failed
(and not a result of improper installation) we are willing to diagnose the failure, however, it will be the customer’s
responsibility to utilize warranty protocol to obtain a replacement part including any shipping or freight charges and it
is the customer’s responsibility to reimburse Parish RV Repair for service call fees and time and labor. If the warranty
does not provide compensation for service call and/or time and labor, it will be the customer’s responsibility to
compensate Parish RV Repair at the time of service for the service call, time, labor, shipping or freight charges
associated with replacement of defective parts.
Payment for service call, time and labor will be required at the time of service and is the customer’s responsibility to
obtain reimbursement from the manufacturer or warranty company for any service call(s), time and labor associated
with warranty protocol on manufactured parts unless otherwise stated or instructed through the warranty company.
Rates:
Call out fee $150
Labor $165 1hr minimum
Emergency Callout Labor $200 1HR minimum
Mileage $3.25 per mile after 10 miles from the shop 16236 Lessie Ln, Prairieville LA
Payments:
1. As always, Cash is King.
2. Check (cashiers, bank check, personal check – just not a bounced check :-P)
3. Card (4% transaction fee for card transactions. Will be sent a link for payment – sorry, everyone wants
their hand in the cookie jar)
Late Payment Fee Charge:
All payments must be paid in full within 4 days of receiving final invoice. Payments made
beyond 4 days will be subjected to a 15% additional late fee, as well as a recurring 15% late fee
weekly beyond due date.
I (Customer) hereby authorize the repair work to be done along with necessary materials. The service technician may
operate the vehicle for purposes of testing, inspection or delivery. An expressed mechanic’s lien is acknowledged on
the vehicle to secure the completion of repairs thereto. Any Parish RV Repair Technician will not be held responsible

for loss or damage to vehicle or articles left in vehicle in case of fire, theft, accident or any other cause beyond the
service technician’s control. By scheduling this appointment, the customer submits this appointment and agreement
to all Electronic Signature Acts, in perpetuity.
